During your initial appointment, we will review your medical history, discuss your concerns, perform a comprehensive evaluation, and create a personalized treatment plan tailored to your needs.
We proudly accept most major insurance plans and welcome patients of all ages.
Patient Forms
To save time during your visit, we encourage you to complete your patient forms before arriving at the office.
Please bring completed forms with you to your appointment if possible.
What to Bring to Your First Appointment
To help us provide the best possible care, please bring the following items:
- Insurance card(s)
- Photo identification
- List of current medications, including dosage and frequency
- Relevant medical and surgical history
- Recent imaging, X-rays, MRI reports, or test results
- Referral information, if required by your insurance plan
We recommend arriving 15 minutes early to allow time for any additional paperwork.
Insurance & Payment Information
Godoy Foot & Ankle Center accepts most major insurance plans. Patients are responsible for any co-payments, deductibles, co-insurance amounts, or services not covered by their insurance provider.
If you have questions regarding accepted insurance plans, billing, or payment options, our team is happy to assist you.

During your appointment, our podiatrists will:
